PROGRAM DIKLAT PELAUT ATT- V PENINGKATAN

Requirements for seafarers' training participants - V
Maritime Competence enhancement in the area of technical expertise of the certificate holder of a skilled seafaring machine (Able Seafarer Engine / ABE):
1. At least 18 (eighteen) years old;
2. AB SPPK engine certificate (Able Seafarer Engine / ABE);
3. The sail period of 24 months as AB engine (Able Seafarer Engine) on a ship with a main propulsion engine of 750 KW or more;
4. Special skills certificates for seamen (Certificates of Proficiency):
        a. BST certificate;
        b. AFF certificate;
        c.  SCRB certificate;
        d. MFA certificate;
        e. SAT certificate;
5. Health certificate;
6. Birth certificate ;
7. KTP or other valid proof of identity;
8. Pass the selection of candidates for training and acceptance.

PROGRAM DIKLAT PELAUT ATT-IV PENINGKATAN

1. At least 18 (eighteen) years old;
2. Upgrading DP - V Certificate or ATKAPIN - II certificate or SPPK DP - V Certificate Enhancement;
3. The sailing period after having an ATT-V certificate 30 (thirty) months or having an ATT-V Management certificate with a sailing period of 12 (twelve) months;
4. The holder of ATKAPIN-II Certificate has a sailing period of 60 (sixty) months, and after completing the DP-IV, the technician must return to complete the sea practice (PRALA) as part of the DP-IV engineering to obtain a 12 (twelve) month sailing period as a cadet documented in an approved training record book (part of the approved training record book) and part of the sailing period has carried out service duties in the engine room, under the supervision of a competent skipper or officer with a period of at least 6 (six) months;
5. Special skills certificate for seamen:
     a.  BST certificate;
     b.  AFF certificate;
     c.  PSCRB certificate;
     d.  MFA certificate;
     e.  SSO certificate;
     f.  SAT certificate ; and
     g. Other seafaring skills certificates.
6. Health certificate;
7. Birth certificate ;
8. KTP or other valid proof of identity;
9. Pass the selection of candidates for training and acceptance.

PROGRAM DIKLAT PELAUT - III (ATT - III)

(PK.303/16/17/DK-17)

1. Photocopy of KTP 2 sheets and show the original
2. Photocopy of 2 birth certificate and show the original
3. Print out according to Seafarer Code which can be seen on the website pelaut.dephub.go.id (original and photocopy of 1 sheet)
4. Valid receipt of education and training payments from the Bank, addressed to the BNI account number: 0165427911-Bend (original and photocopy of 1 sheet)
5. Endorsement (confirmation of seafarer's diploma) (original and photocopy of 2 pieces)
6. Company certificate: Sign On / Off or sea work agreement or work mutation (original and photocopy of 1 sheet)
7. Photograph 3x4 = 15 pieces, Photograph 2x3 = 4 pieces - Background color: red, shirt: white, tie: plain black
8. Photocopy of seaman's book (must have been certified off at the time of registration) (original and photocopy of 1 sheet)
9. Seaman Skills Training (BST, SCRB, MFA, MC, AFF, ERM, SSO) 2 copies of legalized copies
10. Legalized and ORIGINAL photocopy, sailing period of 30 months as a watch keeping enginer officer on a ship with a driving force of 750 KW (seventy five kilo watts) or more (original and photocopies of 1 sheet)
11. ORIGINAL diploma and photocopy of ATT-IV (STCW 78/95) AMK PI certificate (STCW 78) ORIGINAL (original and photocopy of 2 sheets)
12. Legalized copy of STTPK (Surat Tanda Tamat Pendidikan Kepelautan * TPK-IV (DP-IV) (original and 2 pieces of legalized photo)
13. All UPGRADING requirements ar3e scanned in the form of a disc (burning) and submitted together with filing at the Upgrading Counter
14. Family card (original and 2 pieces of copy)

PROGRAM DIKLAT PELAUT - II (ATT - II)

(PK.303/16/16/DK-17)

1. Photocopy of KTP 2 sheets and show the original
2. Photocopy of 2 birth certificate and show the original
3. Print out according to Seafarer Code which can be seen on the website pelaut.dephub.go.id (original and photocopy of 1 sheet)
4. The registration form that has been filled in completely and correctly
5. Health Certificate (eyes and ears), using the blank specified from the designated hospital (blank letter from the organizer of education and training) (original and photocopy of 1 sheet)
6. Receipt of legal education and training payments from the Bank, addressed to the BNI account number: 0165427911-Bend (original and photocopy of 1 sheet)
7. Endorsement (confirmation of seafarer's diploma) (original and photocopy of 2 pieces)
8. Original ATT-III (STCW 78/95) professional diploma and photocopy, AMK A diploma (STCW - 78) legalized copy (original and 2 copies legalized)
9. Company certificate: Sign On / Off or sea work agreement or work mutation (original and photocopy of 1 sheet)
10. Pass photo 3x4 = 15 sheets, passport photo 2x3 = 4 sheets - Background color: red, shirt: white, tie: plain black
11. Photocopy of seaman's book (must have been certified off at the time of registration) (original and photocopy of 1 sheet)
12. Training for seafarers' skills (BST, SCRB, MFA, MC, AFF, ERM, SSO) 2 copies of legalized copies
13. Legalized copy of STTPK (Surat Tanda Tamat Pendidikan Kepelautan) * TPK-III (DP-III) (original and 2 pieces of legalized photo)
-  Sailing period of 36 (thirty six) months (ORIGINAL and photocopied) as a guard engineer in a manned or unmanned engine room, with a driving force of 750 (seven hundred and fifty) KW or more
- Sailing period of 24 (twenty four) months (ORIGINAL and Photocopy) as machinist II (second engineer) in the driving force of 750 (seven hundred and fifty) KW in the local area.
14. All UPGRADING requirements ar3e scanned in the form of a disc (burning) and submitted together with filing at the Upgrading Counter
15. Family card (original and 2 pieces of copy)

PROGRAM DIKLAT PELAUT - I (ATT - I)

(PK.303/16/15/DK-17)

9. General diploma of at least D.III or Academy of Shipping or D IV Sailing or Strata A or STTPK Crash (photocopy of 2 copies)

11. Pass photo 3x4 = 15 sheets, passport photo 2x3 = 4 sheets - Background color: red, shirt: white, tie: plain black
12. A statement stating that the prospective participant is willing to follow the provisions in force (the format of the letter is included in the training provider) (original and a photocopy of 1 sheet)
13.  Photocopy of seaman's book (must have been certified off at the time of registration) (photocopy of 2 pieces)
14. ATT-II Diploma (STCW 78 (BS), AMK B Diploma (STCW 78-95) Photocopy of 2 sheets
15. Legalized copies (BST, SCRB, AFF, MFA, MC, SSO, ERM) that have been amended in Manila 2010 2 sheets
16. - Has a sail period of 36 (thirty-six) months as a watchkeeping / Engineer Officer on a commercial ship with a main propulsion force of 750 KW or more (ORIGINAL sheet of Sails report and Copy 1 sheet)
     -  Has a sail period of 24 (twenty four) months including 12 (twelve) months as a machinist 1 (chief engineer) on a ship with a propulsion engine above 3000 KW
17. All UPGRADING requirements ar3e scanned in the form of a disc (burning) and submitted together with filing at the Upgrading Counter
18. Family card (original and 2 pieces of copy)
