1. Registration form (original & photocopy of 1 sheet)

4. Certificate of sail period of at least 24 (twenty four) months as a deck watchkeeping officer on a commercial ship with a size of GT 500 (five hundred Gross Tonnage) of which 12 (twelve) months as Chief Officer on a merchant ship with a size of GT 500 ( five hundred Gross Tonnage) or more (original & photocopy of 1 sheet)
5. Seaman's book (must have been deemed Off at the time of registration) (2 copies)
6. Print out the maritime certificate database (www.dephub.pelaut.go.id) (original & photocopy of 1 sheet)
7. ANT-III diploma (STCW 78/95) / MPB-III diploma (STCW 78) (original & photocopy of 2 pieces)
8. Endorsement (Certification of Seaman Diploma) (Original & photocopy of 2 sheets)
9. STTPK (Letter of Graduation of Marine Education) / TPK-III (DP-III) (Photocopy which is legalized on 2 pieces)
10. Training for seafarers' skills (BST, SCRB, AFF, MFA, MC, Radar-ARPA, ECDIS, BRM, SSO) (Legalized copies of 2 sheets)
11. KTP (Resident Identity Card) (2 copies)
12. Birth certificate / birth certificate (2 copies)
13. Health certificate (eye and ENT) for seafarers designated by the hospital (original & photocopy of 1 sheet)
